Failures rarely start at the surface. In the ground, they begin in a weak layer well below it: a thin seam of soft or disturbed material that looks harmless until it is asked to carry load. Under stress that seam slips, and everything above it goes too. As a geotechnical engineer I spent years finding those layers before they moved. It turns out organisations have them as well.

Every business carries weak layers of its own. Misaligned incentives. Unclear governance. A capability gap nobody has named. A decision path that quietly adds a week to everything. On their own each one looks manageable. Together, under pressure, they compound, and the failure arrives looking sudden when it was years in the making.

The trouble is that leaders watch the surface. Metrics, timelines and dashboards are visible, so they get the attention. The weak layers sit underneath, out of sight, and they are easy to leave alone because confronting them means reopening legacy decisions, power dynamics and a few uncomfortable truths. Avoiding that feels cheaper. It is not. Weak layers do not stay weak. They widen.

Three signals help you find them early:

Recurring friction: a small problem that keeps returning is usually a symptom of something deeper.

Emotional language: when capable people describe work as "stuck", "conflicted" or "impossible", they are pointing at a pressure point without naming it.

Incentives: most failures trace back to an incentive nobody designed on purpose and nobody wants to discuss.

The engineering discipline is not to eliminate every flaw. Real ground is never perfect, and neither is any organisation. The discipline is to know where the weak layers are, understand what load they can carry, and strengthen the ones that sit under the heaviest stress before the stress finds them first.

Good strategy work is often less about adding something new and more about this: locating the seam that will slip, and reinforcing it while the choice is still yours to make.
